Wisdom teeth are scientifically called third molars, the third set of molars that most people get in their late teens or early twenties.

Mostly they are misaligned and require removal. They may position themselves horizontally, angulated towards or away from the second molars, or angled inward or outward. Poor alignment of wisdom teeth can crowd or damage the adjacent teeth, jawbone or nerves.

Wisdom teeth also can be impacted; they are enclosed within the soft tissue. Partial eruption of the wisdom teeth allows an access for bacteria to enter the tooth and cause infection which results in pain, swelling, jaw stiffness and general illness. Partially erupted teeth are also more prone to tooth decay and gum disease because it is hard to reach 3rd molar.

Position of third molar makes brushing and flossing difficult. Position of third molar can be evaluated by taking X RAY. Based on the position of third molar the treatment is suggested.

Extraction of third molar is suggested to avoid more problems i.e., PAIN and SWELLING. This is done to avoid a more painful or more complicated extraction.

A Wisdom tooth that is fully erupted through gum can be extracted easily as any other normal tooth.

The complication of wisdom tooth depends on the position of third molar .Position may be:

  • Mesioangular.
  • Partially erupted.
  • Vertical.
  • Horizontal.
  • Bucco angular.
Removal:

Wisdom tooth removal is done under local anesthesia. The total area surrounding third molar is anesthetized. A small incision is done to expose the teeth to be removed and if the tooth if firmly anchored to the bone a little bit of sectioning and bone removal might be required to be done by the surgeon in order to loosen the tooth so as to deliver the tooth out of the socket without causing any damage to the adjacent tooth structures. Post extraction of the tooth the area is completely debrided of all fragments and sutures are placed.

Recovery:

After removal the speed of recovery depends on the degree of difficulty of the extraction. You should not consume solid foods, coffee, soda or hot beverages in first few days following your procedure. You shouldn’t brush your teeth for first day of recovery. You can resume your normal activities after first day in most cases.

Bleeding:

Bleeding may occur for several hours after tooth extraction. To control the bleeding a piece of clean moist gauze piece is placed over the empty tooth socket and bite firmly. Cold application is suggested. Intake of cold and warm food is suggested. Moistened tea bag is an effective alternative. The tannic acid in tea helps healing blood clots to form. Avoid rinsing or spitting for 24 hours and avoid sucking habits and hot liquids. Avoid beverages through straw and smoking cause these activities dislodge the clot causing a DRY SOCKET to develop.

Dry Socket (DELAYED Healing):

It is a common complication which occurs when blood clot has failed to form.

Without clot formation the healing will be delayed. Dry socket occurs 3 or 4 days following the extraction and is accompanied by pain. Pain starts from dull to moderate to severe and lasts for mostly 3 to 4 weeks.

Paresthesia:

It is a rare complication of wisdom teeth extraction. Wisdom teeth sometimes entrapped in the jaw bone are often close to nerves can be bruised or damaged during tooth removal. The result is numbness of tongue, lip or chin can lasts for days, weeks, months or may be even permanent.

Facial Swelling:

Seen in the area of extracted tooth. Cold application is suggested. After 24 hours rinse your mouth with warm salt water after meals and before bed. Watch for sign of dry socket. Complete healing does not occur for a few weeks to months. Usually,within 2 weeks healing takes place and hence normal chewing habits can be followed.

Medication:

For pain relief medications such as Ibuprofen or Diclofenac + Serratiopeptidase are given. Antibiotics are prescribed and should be taken until the full course of treatment is completed.
